2022 PLAYER WAIVER:                                                                 In consideration of participating in the Hill Country 2022 summer travel season, the player named below and the parent or guardian do hereby agree for ourselves, our heirs, executors and administrators, to release, hold harmless and forever discharge the city of Dripping Springs, Founders Park, DSHS, the city of Lakeway, Dragon Park, the City of Austin, West Austin Youth Association, Dick Chalmers Park, ATX Sandlot, ATX Lacrosse LLC., Hill Country Lacrosse LLC, Bill Cafferata, their officers, staff, administrators, volunteers, sponsors and representatives and assigns, for and against any and all claims, actions, cause of actions, suits, judgments, and demands whatsoever arising directly or indirectly in connection with the player’s participation in the Hill Country Lacrosse 2022 summer travel season. I am fully aware and appreciate the risks, including the risk of a catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in a lacrosse event. By signing below, I acknowledge that I have read and understand this form and further understand the terms herein are contractual and not a mere recital. I/we being the legal guardians of the applicant authorize the staff of the Hill Country Lacrosse 2022 summer travel season and their agents permission to request treatment as necessary to ensure the wellbeing of our dependent. I certify that he is in good health and able to participate in the scheduled games. *

2022 WEATHER CANCELLATION POLICY:                                                                     Summer weather is very unpredictable and this may jeopardize practices and/or games. If practices are cancelled due to unsafe weather conditions, we will do everything in our power to make them up on a different date. If tournament games are cancelled, we will be following the individual tournaments cancellation guidelines. If a tournament is cancelled altogether, we will do everything we can to make up those games locally through play days with other teams. If any of the above cancellations occur, there will not be a refund issued. *
